If you are a current patient of Case Western Reserve University School of Dental Medicine Dental Clinic, register for My Dental Chart access online by clicking here or on the Register link on the bottom of this page.
Representatives of Current Patients
If you are an authorized representative (such as a parent, legal guardian, etc.) of a current patient who will be filling out forms on their behalf, request access to My Dental Chart by calling the Dental Clinic at 216.368.3200 during regular business hours (Monday-Friday, 8:30 a.m. to 5 p.m. ET).
How Do I Reset My Password or Username?

If you previously had access but forgot your username or password, there are links to reset both on the login page.

If you represent a current patient, but are not a patient yourself, you will need to call the Dental Clinic at 216.368.3200 during regular business hours (Monday-Friday, 8:30 a.m. to 5 p.m. ET) to reset the password.
How Do I Get Support?
If you are experiencing other issues with the website, first try clearing your cache and cookies. Refer to your web browser's documentation for instructions. My Dental Chart will also not work in Internet Explorer.
